Antiangiogenic drugs, including vascular endothelial growth factor inhibitors (VEGFis), while effective anticancer agents, unfortunately often produce unwanted side effects, including hypertension and vascular toxicity. Blood pressure elevations have been observed in patients treated with PARP inhibitors, a class of medications used to combat ovarian and other cancers. Cancer patients given both olaparib, a PARP inhibitor, and VEGFi demonstrate a reduced possibility of experiencing elevated blood pressure. The fundamental molecular mechanisms remain shrouded in mystery, but PARP-regulated transient receptor potential cation channel, subfamily M, member 2 (TRPM2), a redox-sensitive calcium channel, may have a substantial influence. We aimed to uncover if PARP/TRPM2 is a player in VEGFi's inducement of vascular dysfunction, and if obstructing PARP activity might improve the vasculopathy associated with VEGF interference. In the methods and results, human vascular smooth muscle cells (VSMCs), human aortic endothelial cells, and wild-type mouse mesenteric arteries were examined. Axitinib (VEGFi), or axitinib (VEGFi) in addition to olaparib, was used to treat cells/arteries. Proinflammatory marker elevation in axitinib-treated VSMCs was diminished by interventions targeting reactive oxygen species and PARP-TRPM2. Human aortic endothelial cells treated with both olaparib and axitinib exhibited nitric oxide levels mirroring those found in cells stimulated by VEGF. PARP and TRPM2 are implicated in the vascular dysfunction triggered by Axitinib; their inhibition effectively diminishes the injurious influence of VEGFi. Our research suggests a potential mechanism whereby VEGFi-treated cancer patients might experience reduced vascular toxicity thanks to PARP inhibitor use.
A recently recognized tumor entity, biphenotypic sinonasal sarcoma, presents with unique clinicopathological features. Within the sinonasal tract, biphenotypic sinonasal sarcoma, a rare, low-grade spindle cell sarcoma, is found almost exclusively in middle-aged women. Individuals diagnosed with hypertrophic cardiomyopathy (HCM) displaying sarcomere gene mutations and myocardial fibrosis tend to have a less favorable clinical course. Tohoku Medical Megabank Project Through the combination of histopathological evaluation and cardiac magnetic resonance (CMR) assessment, this study aimed to characterize the correlation between sarcomere gene mutations and myocardial fibrosis. This study involved 227 patients with hypertrophic cardiomyopathy (HCM), who had undergone surgical treatment, genetic testing, and cardiac magnetic resonance imaging (CMR). Retrospective analysis of basic characteristics, sarcomere gene mutations, and myocardial fibrosis, as identified by CMR and histopathology, is presented here. Based on our study, the average age of participants was 43 years, with 152 patients (670%) identifying as male. A positive sarcomere gene mutation was found in a total of 107 patients, representing 471%. A significantly elevated myocardial fibrosis ratio was observed in the late gadolinium enhancement (LGE)+ group, compared to the LGE- group (LGE+ 14375% versus LGE- 9043%; P=0001). In patients with hypertrophic cardiomyopathy (HCM) accompanied by sarcopenia (SARC+), a significant predisposition for fibrosis was observed, as evidenced by both histopathological examination (myocardial fibrosis ratio 15380% versus 12465%; P=0.0003) and cardiac magnetic resonance (CMR) imaging (LGE+ 981% versus 842%; P<0.0001; LGE quantification 83% versus 58%; P<0.0001). Analysis using linear regression demonstrated a relationship between histopathological myocardial fibrosis and both sarcomere gene mutation (B = 2661; P = 0.0005) and left atrial diameter (B = 0.240; P = 0.0001). The MYH7 (myosin heavy chain) group demonstrated a statistically significant (P=0.0019) increase in myocardial fibrosis ratio compared to the MYBPC3 (myosin binding protein C) group; the respective ratios were 18196% and 13152%. Among hypertrophic cardiomyopathy (HCM) patients, those with positive sarcomere gene mutations manifested more myocardial fibrosis, in contrast to patients without these mutations.
