Intensive care medicine (ICM) survivors frequently face a collection of sequelae after their stay, and the Coronavirus Disease 2019 (COVID-19) pandemic has added significant new obstacles. ICM memories are particularly influential; in contrast, delusional memories are associated with adverse post-discharge consequences, including a delay in returning to work and sleep issues. The increased risk of experiencing delusional memories under deep sedation has led to a change in preference towards lighter sedation. Limited accounts exist regarding post-intensive care unit memory in individuals with COVID-19, and the role of deep sedation in these recollections has yet to be thoroughly examined. Therefore, we embarked on a study to measure ICM-memory recall in COVID-19 survivors and to analyze its link to deep sedation experiences. A Portuguese University Hospital evaluated adult COVID-19 Intensive Care Unit survivors, admitted between October 2020 and April 2021 (second/third waves), one to two months post-discharge. The ICU Memory Tool assessed real, emotional, and delusional memories experienced by the patients. Of the 132 patients in the study, 67% were male, with a median age of 62 years. The patients' APACHE-II scores were 15, their SAPS-II scores were 35, and their average length of stay in the Intensive Care Unit was 9 days. A significant portion, approximately 42%, of the patients experienced deep sedation, lasting a median of 19 days. Participants predominantly reported accurate memories (87%), along with emotional memories (77%), with a considerably smaller number (364) experiencing delusional recollections. Patients undergoing deep sedation reported significantly fewer verifiable memories (786% vs 934%, P = .012) and a notable surge in delusional memories (607% vs 184%, P < .001). Analysis of emotional memory retention revealed no significant difference (75% vs 804%, P=.468). Deep sedation displayed a statistically significant, independent connection with delusional memories in multivariate analysis, increasing the likelihood of these memories approximately six-fold (OR = 6.274; 95% CI = 1.165-33.773, P = .032), whereas it had no impact on the recall of real-life experiences (P = .545). Memorable moments, imbued with feeling or sentimentality (P=.133). By studying critical COVID-19 survivors, this research uncovers a substantial, independent correlation between deep sedation and the frequency of delusional recollections, contributing insights into potential adverse effects on ICM memories. Although additional research is necessary to validate these results, they imply a preference for sedation-minimizing strategies, aiming for improved long-term recovery outcomes.
The role of attention in prioritizing environmental stimuli is pivotal in shaping overt decision-making. Research suggests a link between the size of paired rewards and prioritization, specifically, stimuli indicative of substantial rewards are more likely to attract attention than stimuli indicating smaller rewards; this attentional bias is posited as a contributor to the development of compulsive and addictive tendencies. To comprehend Chinese nurses' resilience in confronting death, examining how their perception of death's meaning, their outlook on life, and traditional Chinese culture converge. In the recruitment effort at six tertiary hospitals, 1146 nurses were involved. Participants systematically completed the Coping with Death Scale, the Meaning in Life Questionnaire, and the uniquely devised Death Cognition Questionnaire. Multiple regression analysis indicated that the pursuit of meaning, the understanding of a meaningful death, exposure to life-and-death education, cultural factors, a sense of purpose, and the tally of patient deaths experienced during a career elucidated 203% of the variance in the ability to cope with death. Without a profound understanding of death, nurses may lack the necessary resources to effectively navigate the experience of death, their capacity for coping influenced by distinctive perspectives on death and the search for meaning within the framework of Chinese traditional culture.
